PDF files are easily opened and read using free software from the Adobe Products website.
If you don't have Adobe Reader loaded on your computer, you can easily obtain the software. Just copy and paste the Adobe Reader website address into the "Address" bar at the top of this webpage.

Follow the directions to download your free PDF reader.
Then click on the words "Board Survey Instrument" below.
That's all there is to it! If something goes wrong and you need help, call Gary at 217 737-3359 and he will fax you a copy of the survey instrument.
